Complaint Form
If you are not satisfied with a service provided by the Mental Health Foundation or are concerned with the health, conduct, or performance of a registered or unregistered healthcare provider, you can address a complaint to the MHF Incidents & Complaints Committee (INCOM).
Before filing a complaint, try talking to the department supervisor, this is often the quickest and easiest way to address your concerns or resolve the problem. The committee meet quarterly and for urgent matters more often, the secretary of the committee will confirm receipt of you complaint and notify you when to expect an answer. Complaint Form
